This National Public Health Week, we’re highlighting how better health means more access to care management.

On MyWayfinder we’re encouraging users to take control not just of their wellness journeys, but their health journeys as well, with their healthcare provider’s patient portal. Thanks to our partners at the University of Rochester Medical Center (URMC), we’ve added a helpful video to show how to take control of your health.

Organizations can help spread the word on patient portals.

To equip community support navigators, URMC is running the Digital Health Equity Navigation Training (DHENT) project. Community-based organizations (CBOs) join DHENT to learn how to teach their clients to make the most of patient portals like MyChart and MyCare.

Tanisha Gamble-Snead, LMSW, DHENT’s Project Manager and Senior Project Manager at URMC’s Health Equity Program Support Office (HEPSO), noted the importance of health education at every age. “Witnessing the cross-generational education has been a win!” she said of her experience facilitating trainings. “Watching generations engage and educate around a common topic is beautiful.”
